Tuners can be used as direct replacements on most PRS Core guitars with larger style PRS / Schaller M6 Phase I tuners. No modifications necessary.

These seriously brought my '87 PRS Standard back to life. I had this guitar in a case for years because if you could even get it in tune, it would immediately fall out when you started playing. $200 bucks and and half hour made this my main instrument again. Unless there is some magic in the new strings I put on, or a serious placebo affect (I will admit this is a slight possibility), not only do I stay in tune now, but the sustain and snappiness of the guitar has improved a noticeable amount. I am thrilled with this purchase. Consider my stars 4.75, because the holes don't perfectly align. I assume this is a result of human error in a pre-factory guitar. A couple of tuners had to be at a slightly off angle to not modify the headstock, and the shared DG hole only really holds onto one of the tuners and I had to get a cute with that to not modify anything. I would re-buy these 1000 out of 1000 times, though. Just a word to the wise if you are putting these on a really old core PRS.
